These are the workers who have retired from Department of Energy facilities such Lawrence Livermore National Lab, Sandia National Lab Lawrence Berkeley National Lab, Stanford Linear, etc. We also serve Federal Workers such as US Postal Workers.Unlike many home health agencies that require multiple short visits per shift, Hallway Healthcare typically assigns you to one patient per shift, allowing for more focused, consistent, and meaningful care.We offer flexible schedules, competitive pay, and 24/7 available support.Schedule/Shifts:LOCATION: Sacramento, CA - close to i80 and Sacramento AveAvailable Shifts: 12 hour shifts 7am-7pm or 7pm-7amFull-time, Part-time, Per diem availablePay rates: $45.00-$50.68/hrWe offer competitive shift differentials, including an additional $5/hour for 8-hour shifts and $10/hour for shifts under 8 hours. For shifts of 8 hours or more, overtime (time-and-a-half) is paid for all hours worked beyond 8 in a day.Other locations and shifts available!BENEFITS:- Medical Insurance- Dental Insurance- Vision Insurance- 401k-401k match up to 3%- Mileage 1 way after 30 miles**Insurance available for full-time employees. 401k eligibility after clocking 1,000 hours.APPLY TODAY!Required Education, License and certification:Graduated from an accredited school of professional nursing.Current California RN/LVN LicenseCurrent BLS CertificateResponsibilities/essential functions: The person in this position must be able to perform the following essential job functions with or without reasonable accommodations.Demonstrate efficient teamwork with the staff.Demonstrate organizational and time management skills.Support quality improvement practices.Perform nursing procedures according to agency policy and procedures.Work under the direction of a RN Case Manager and Director of Nursing.Monitor reactions and patient progress using observation, assessment, and evaluation skills.Educate patients and family members according to disease process, medications, POC, treatment options, and home care procedures according to the plan of care.Report to physician and RN Case Manager of adverse findings.Follow state regulations.Shall complete clinical notes for each visit, which must be incorporated into a record on a daily basis.Coordinate and monitor patient care and services.Comply with HIPAA regulations.Follow infection control policy.Light housekeeping, meal preparation.Transport patients to doctors appointments, grocery stores or pharmaciesDocument skilled visits accurately and in accordance with established guidelines. Documentation must be completed by the end of each shift. Some homes will be clean, neat, and maintained at a comfortable temperature.
