Registered Nurse - Home Health & Hospice Job Category : Nursing Requisition Number : REGIS002074 Full-Time On-site Locations Showing 1 location Grand Lake Home Health St. Marys, OH 45885, USA Description Hours of Job FT - 8 a.m. - 4:30 p.m. Monday - Friday Every 8th-9th Weekend Coverage (secondary on-call) One weekend day/year (primary on-call) 3-4 weekday nights/month ( Primary On-call) 1-2 Holidays per year (Primary On-call) Duties and Key Responsibilities Responsible for the delivery of patient care through the Nursing Process of assessment, planning, implementation, and evaluation. Directs and guides patient teaching and activities of Nursing Ancillary Personnel. Assumes a lead role in patient teaching. Places the patient at the center of care practices. Collaborates and interacts positively with the interdisciplinary team to provide quality care and achieve desired outcomes. Requirements Flexibility in scheduling to meet department needs. Education/Certifications Graduate of an accredited school of nursing. Licensed RN. Experience Previous Home Health/Hospice experience is preferred but not required.
