- Conduct Social Work assessments to determine the psychosocial needs, preferences, and goals of the participants.
- Actively participate in IDT meetings to develop participant care plans based on assessment findings.
- Deliver and document social work interventions as outlined in participants' care plans, including arranging necessary resources and services, providing counseling, and offering case management.
- Collaborate with the primary care physician and other members of the care team to ensure smooth care transitions between settings.
- Initiate, coordinate, and facilitate care conference meetings to enhance care coordination among care team members, participants, and their support network.
- Provide discharge planning for participants disenrolling from the program.
Job Requirements:
- Masters Degree in Social Work (MSW) required.
- Minimum of one (1) year of experience with a frail or elderly population.
- Previous experience coordinating and facilitating care conference meetings.
- Preferred previous experience assisting individuals with behavioral health and substance abuse issues.
- MSW Required
- 2+ to 5 years of experience.
- Seniority Level: Associate
- Management Experience Required: No
- Minimum Education: Master's Degree
- Willingness to Travel: Occasionally
