Position SummaryExperienced Licensed Practical Nurse (LPN) provides skilled nursing care under RN supervision, exercising clinical judgment, implementing care plans, administering treatments, and supervising unlicensed staff in accordance with the State Nurse Practice Act.Key ResponsibilitiesConduct focused patient assessments and report significant condition changesImplement and evaluate nursing interventionsAdminister medications and perform treatments as orderedDocument care accurately in PointClickCare (PCC)Communicate pertinent clinical information to RNs, physicians, and care team membersSupport discharge planning and patient/family educationDelegate and supervise unlicensed staff per policy and regulationMaintain medication security, including controlled substance countsParticipate in shift handoff and interdisciplinary coordinationAdhere to safety, infection control, HIPAA, and regulatory standardsRequired QualificationsGraduate of an approved practical or vocational nursing programCurrent LPN license issued by the State Board of NursingExperience in rehabilitative or geriatric nursing preferredAbility to read, write, speak, and understand English to ensure patient safetyVerification of TST (tuberculin skin test) as required by state law and company policyCurrent BLS/CPR certification (required for SNFs and Behavioral Centers; ALFs/ILFs per state regulation)
