About the Role:

The Neighborhood Coordinator LPN plays a critical role in delivering high-quality nursing care and coordinating health services within community settings. This position focuses on managing patient care plans, providing direct nursing services, and acting as a liaison between patients, families, and healthcare providers to ensure comprehensive and continuous care. The role requires proactive engagement with community members to promote health education, monitor patient progress, and address any emerging health concerns promptly. The Neighborhood Coordinator LPN also collaborates with interdisciplinary teams to optimize patient outcomes and supports public health initiatives tailored to the community's needs. Ultimately, this position aims to enhance the overall well-being of individuals in the neighborhood by integrating clinical expertise with compassionate care and effective coordination.

Minimum Qualifications:

• Current and valid Licensed Practical Nurse (LPN) license in the state of practice within the United States.
• High school diploma or equivalent; completion of an accredited practical nursing program.
• Basic Life Support (BLS) certification.
• Minimum of one year of clinical nursing experience, preferably in community health or home care settings.
• Strong knowledge of nursing principles, patient care techniques, and infection control standards.

Preferred Qualifications:

• Experience working in community health, public health, or home health nursing.
• Additional certifications such as Certified Community Health Worker (CCHW) or similar.
• Familiarity with electronic health records (EHR) and healthcare documentation software.
• Excellent communication and interpersonal skills tailored to diverse populations.
• Ability to speak a second language relevant to the community served.

Responsibilities:

• Provide direct nursing care to patients in community and home settings, including administering medications, wound care, and monitoring vital signs.
• Develop, implement, and update individualized care plans in collaboration with patients, families, and healthcare teams.
• Coordinate communication between patients, families, healthcare providers, and community resources to ensure seamless care delivery.
• Conduct health assessments and screenings to identify patient needs and potential health risks within the neighborhood.
• Educate patients and community members on disease prevention, health maintenance, and management of chronic conditions.
• Document patient care activities accurately and maintain compliance with healthcare regulations and organizational policies.
• Participate in community outreach programs and public health initiatives to promote wellness and access to healthcare services.
• Monitor patient progress and report any changes in condition to the supervising Registered Nurse or healthcare provider.

The required skills enable the Neighborhood Coordinator LPN to deliver safe and effective nursing care by applying clinical knowledge and technical nursing procedures daily. Strong communication skills are essential for educating patients and coordinating with healthcare teams, ensuring that care plans are understood and followed. Organizational skills help manage multiple patients’ needs and documentation requirements efficiently, maintaining compliance with healthcare standards. Preferred skills such as proficiency with electronic health records streamline documentation and improve information sharing among providers. Additionally, cultural competency and language skills enhance the ability to connect with diverse community members, fostering trust and improving health outcomes.
