TITLE: Geriatric Support Services and Long-Term Care Services Coordinator (SCO/OC Case Manager)

This is a full-time (35 hr/wk) role based out of our Greenfield office, serving the Franklin County, and North Quabbin area. We are dedicated to the well-being of older adults, individuals with disabilities, and caregivers. GENERAL DESCRIPTION

The Case Manager is responsible for developing supportive relationships with older adults and people with disabilities, fostering independence, and serving as a central source of support and resources. This position is also responsible for the evaluation, needs assessment, and overall implementation of Geriatric Support and Long-Term Care Services for eligible applicants. Responsibilities include dissemination of information and making appropriate referrals, setting up and monitoring a service plan for each consumer, and serving as an advocate. 

DUTIES & RESPONSIBILITIES:

1. Conduct in-person comprehensive, person-centered assessments with consumers, care partners, and health-care professionals.
2. Develop, implement, and update a service plan with the consumer in collaboration with their formal and informal support systems
3. Authorize internally funded services.
4. Make appropriate referrals to other community-based agencies and governmental organizations
5. Collaborate with internal and external providers to ensure adequate coordination of service plans (to include meetings and telephone contacts).
6. Adhere to documentation standards and maintenance of consumer records according to regulations.
7. Assist clients, families, caregivers, and informal support in problem-solving. Facilitate team meetings and case/family conferences as needed.
8. Maintain up-to-date knowledge of information about services available to older adults and individuals with a disability in the service area.
9. Serve as an advocate for consumers to obtain other needed services.
10. Participate in interdisciplinary team meetings as needed.
11. Act as a representative of LifePath that times when directed to do so.
12. Provide office coverage as assigned.
13. Participate in staff meetings and trainings as required.
14. Assisting training new staff by allowing them to shadow visits and by sharing information and tips.
15. Other duties as assigned by immediate supervisor, other designated supervisory staff, or designated member of the Management Team.

QUALIFICATIONS:

Bachelor’s Degree preferred and 2-3 years of previous experience in health or human services required. License in Social Work is a plus. 

Skills and Experience:

• Willingness and sensitivity for working with older adults and individuals with disabilities.
• Strong documentation skills and familiarity with electronic health records (EHR) or care management platforms and Microsoft programs.
• Ability to work independently and as part of an interdisciplinary team.
• Exceptional interpersonal and communication skills
• Capacity to complete comprehensive needs assessments using observation, listening, and interviewing skills.
• Ability to solve problems and find solutions. 
• Flexible and able to meet multiple, and changing, program demands.
• Familiarity with or willingness to learn about health and human services programs.
• Demonstrates cultural humility and cultural responsiveness.

The GSS/LTCS Coordinator reports to the Senior Care Options/One Care Program Director

ESSENTIAL FUNCTIONS:

Physical:

• Visual, speaking, auditory, and mobile capacity necessary.

• Ability to sit or stand for extended periods of time.
• Occasional reaching and grabbing objects with both hands, twisting of hand and wrist, and pushing and pulling of objects.
• Occasional bending, squatting, and twisting to perform work functions.
• Occasional capacity to lift up to 25 pounds.

Mental:

• Ability to understand and/or interpret complicated program instructions, regulations, and laws.
• Capacity to learn complicated computer software.
• Capacity to deal rationally and calmly with varying personalities and situations.
