As a Clinical Pharmacist, your expertise will directly support the health of the community. What You’ll Do Collaborate with providers to deliver medication reconciliation, pain management, and chronic disease support. Counsel patients on safe, effective medication use — both in clinic and at community outreach locations. Monitor therapeutic progress and document care in the electronic health records (EHR) system. Provide education for medical residents, providers, and the clinical team. Support programs like diabetes care, tobacco cessation, and anticoagulation therapy. Research and recommend evidence‑based formulary changes to enhance access and care. What You Bring Doctor of Pharmacy (PharmD) and current WA State Pharmacist license, and must meet one of the following requirements: Completion of an ambulatory practice pharmacy residency program. 2 years of clinical ambulatory experience. 10 years of pharmacy practice experience.
