Home Health Occupational Therapist (OT) Provide personalized occupational therapy to patients in their homes, helping them regain independence and perform daily activities. Collaborate with a multidisciplinary team, assess patient needs, set therapy goals, and document progress. Assist with exercises, recommend adaptive equipment, assess fall risks, and educate patients and families on care plans. Supervise and guide Certified Occupational Therapy Assistants.
