LTAC patients typically have chronic, serious conditions that require continuous monitoring and treatment. LPNs in LTAC settings play a crucial role in administering care, monitoring patient progress, and assisting with long-term rehabilitation while ensuring patients' comfort and safety. Key Responsibilities :
- Patient Care and Monitoring :
- Monitor and record vital signs, including blood pressure, temperature, pulse, and respiration, frequently throughout the shift.
- Observe and document patients' conditions, noting any changes in health status and reporting abnormalities to the supervising Registered Nurse (RN) or physician.
- Administer prescribed medications (oral, injectable, or IV) and treatments as directed by the physician, ensuring correct dosages and monitoring for any adverse reactions.
- Assist patients with activities of daily living (ADLs) such as bathing, dressing, feeding, and mobility.
- Provide comfort measures to patients, such as pain management, and ensure adequate hydration and nutrition.
- Wound and Skin Care :
- Assist with wound care and dressing changes, particularly for patients recovering from surgery or injury.
- Monitor for signs of infection or complications related to wounds, surgical sites, or chronic conditions.
- Perform routine skin assessments to prevent pressure ulcers, especially for immobile patients.
- Respiratory Support :
- Monitor respiratory function for patients requiring mechanical ventilation, oxygen therapy, or tracheostomy care.
- Assist in suctioning and maintaining airway patency, as directed by the healthcare team.
- Collaborate with respiratory therapists to monitor respiratory equipment and adjust settings as needed.
- Rehabilitation and Mobility :
- Assist patients with mobility, including turning, repositioning, and walking, as needed, to prevent bedsores and aid in recovery.
- Support physical therapy exercises, ambulation, and rehabilitation programs tailored to each patient's condition.
- Encourage and motivate patients to participate in their rehabilitation plan to regain strength and improve mobility.
- Collaborating with Healthcare Team :
- Work closely with RNs, physicians, respiratory therapists, physical therapists, and other healthcare professionals to implement and adjust care plans.
- Participate in interdisciplinary team meetings to discuss patient progress, goals, and care strategies.
- Communicate effectively with patients, families, and the healthcare team regarding treatment plans, care procedures, and recovery expectations.
- Infection Control and Safety :
- Ensure adherence to infection control protocols, including hand hygiene, sterilization, and safe handling of medical equipment.
- Follow standard precautions when managing patients with communicable diseases or those requiring isolation.
- Monitor patients for any signs of infection or complications related to medical devices (e.g., IVs, catheters, feeding tubes) and notify the healthcare team of concerns.
- Documentation and Reporting :
- Maintain accurate and detailed patient records, including daily assessments, medication administration, treatments, and changes in condition.
- Ensure documentation complies with state and federal regulations, as well as organizational policies (e.g., HIPAA).
- Report any changes in patient status to the RN or physician, ensuring a seamless handoff of care during shift changes.
- End-of-Life Care (if applicable) :
- Provide compassionate care to patients at the end of life, ensuring comfort and dignity during their final days.
- Work with the healthcare team to support families and facilitate decision-making in accordance with patients' wishes.
