Licensed Physical Therapist (Home Health) – Lake Worth Area or Boynton Beach to Delray Area

About the Role:
M.D. Home Health, LLC is seeking a Licensed Physical Therapist to provide in-home care throughout the Lake Worth Area or Boynton Beach to Delray Area . This is a flexible, patient-centered opportunity for a clinician who values autonomy, efficiency, and meaningful patient impact.

You won’t be stuck in a clinic juggling 3 patients at once—you’ll deliver one-on-one care where it actually matters: in the patient’s home.

BONUS OPPORTUNITY: 
Earn up to $1,500 in performance-based bonuses for maintaining a consistent caseload, with additional incentives for servicing high-demand areas.

What You’ll Do:

• Perform in-home evaluations and reassessments to develop individualized plans of care 
• Deliver skilled therapy interventions to improve mobility, strength, balance, and independence 
• Provide fall prevention, gait training, and functional mobility training
• Educate patients and caregivers on home exercise programs and safety strategies
• Communicate with physicians and interdisciplinary team members to ensure coordinated care 
• Complete documentation in a timely manner using our EMR system 
• Manage your caseload efficiently within your assigned territory 

What We’re Looking For:

• Active Florida Physical Therapist License (Required)
• Minimum 1 year of clinical experience (home health preferred, but not required) 
• Strong clinical decision-making and time management skills 
• Ability to work independently in the field 
• Reliable transportation and willingness to travel within assigned coverage area 
• Comfortable with EMR documentation 
• CPR certification
