Job Summary:

Responsibilities:

  Assess patients' physical conditions and develop individualized treatment plans.

  Deliver evidence-based therapeutic interventions to improve mobility, strength, and function.

  Document patient progress and update treatment plans accordingly.
  
  
• 
  Educate patients and their families on exercises, injury prevention, and management strategies.

  Collaborate with healthcare team members to optimize patient outcomes.

  Maintain compliance with licensing, credentialing, and facility policies.
  
  

Qualifications:

• 
  Hold an active physical therapy license with the required state credentials.
  
  
• 
  Demonstrate experience in outpatient rehabilitation and therapeutic modalities.
  
  
• 
  Possess excellent communication skills for patient education and team collaboration.
  
  
• 
  Exhibit strong clinical assessment and documentation capabilities.
