Registered Nurse - Patient Aligned Care Team

Summary Registered Nurse (RN) position located at Sarpy County Clinic in Papillion - NE. The Patient Aligned Care Team (PACT) Outpatient Staff RN is responsible for providing competent, evidence‑based gender‑specific care to primary care patients and oversight of licensed vocational nurses and nursing assistants as appropriate to the setting. Provides direct Veteran care and care coordination to Veterans within the Primary Care Department. Qualifications Basic Requirements: English Language Proficiency In accordance with 38 U.S.C. 7403(f) no person shall serve in direct patient care positions unless proficient in basic written and spoken English Graduate of a school of professional nursing approved by the appropriate accrediting agency and accredited by ACEN or CCNE OR individuals attending a master’s level bridge program who have completed coursework equivalent to a bachelor’s degree in Nursing may become registered as a nurse with a state licensing board prior to completion of the bridge program Upon achievement of a state license the individual may be appointed on a temporary basis and later converted to a permanent appointment upon successful completion and graduation from the bridge program Reference VA Handbook 5005 – Appendix G6 OR in cases of graduates of foreign schools of professional nursing possession of a current, full, active, and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing Credit for foreign nursing education higher than associate degree/entry level requires formal degree equivalency validation from a recognized equivalency evaluation accepted by VA such as International Consultants of Delaware (ICD) Current full, active, and unrestricted registration as a graduate professional nurse in a U.S. State, Territory, Commonwealth, or District of Columbia Graduate Nurse Technician Exception: candidates who meet basic education requirements but do not possess required licensure may be appointed at the entry step of the grade and level on a 120‑day temporary appointment while pursuing licensure (may be extended up to two years on a case‑by‑case basis) Grade Determinations: Scope, Education and Dimension criteria must be met to determine grade assignment and level within a grade. Reference Grade/Level Scope Education Nurse I – Level I delivers fundamental knowledge‑based care to assigned clients while developing technical competencies. Associate Degree (ADN) or Diploma in Nursing with no additional professional nursing required. Nurse I – Level II demonstrates integration of biopsychosocial concepts, cognitive skills, and technical competence in providing care to clients with basic or complex needs. Either an ADN or Diploma in Nursing and 1 year of specialized nursing experience equivalent to Nurse I – Level I; OR a Bachelor of Science in Nursing (BSN) with no additional professional nursing experience required. Nurse I – Level III demonstrates proficiency in practice based on conscious and deliberate planning and self‑directed goal setting for managing complex client situations. Either an ADN or Diploma in Nursing and 2 years of professional nursing experience; OR a BSN and 1 year of professional nursing experience equivalent to Nurse I – Level II; OR a Master’s degree in nursing and no additional professional nursing experience; OR a Master’s in a related field with a BSN and no additional professional nursing experience. Nurse II demonstrates leadership in delivering and improving holistic care through collaborative strategies with others. A BSN with 2 years of professional nursing equivalent to Nurse I – Level III; OR an MSN with one year of specialized nursing experience; OR a Master’s in a related field with BSN and one year of specialized nursing experience; OR a Doctoral degree in Nursing with no professional nursing experience; OR a Doctoral degree in a related field with BSN and no professional nursing experience. Nurse III executes position responsibilities demonstrating leadership, experience, and creative approaches to managing complex client care beyond the immediate practice setting. MSN with 2 years of specialized nursing experience; OR a Master’s in related field with BSN and two years of specialized nursing experience; OR a Doctoral degree in Nursing with one year of specialized nursing experience; OR a Doctoral in a related field with BSN and one year of specialized nursing experience. *Note: Foreign education programs and degrees are not creditable as related degrees. Physical Requirements: Participate in a pre‑employment examination or evaluation as part of the pre‑employment process. Questions about physical demands or environmental factors may be addressed at the time of evaluation or examination. Reference: For more information on this qualification standard – Preferred Experience: 2-3 years of recent clinical RN experience in Primary Care – Ambulatory Care – PACT – Medical‑Surgical – or other outpatient settings. Duties of the Registered Nurse – Patient Aligned Care Team (PACT) include – but are not limited to: Provide evidence‑based, gender‑specific nursing care to Veterans in the Primary Care/PACT outpatient setting. Manage assigned patient panels, utilize clinical registries, and ensure timely access, evaluation, and coordinated care. Deliver health promotion, disease prevention, and chronic disease management education and coaching to patients. Support licensed vocational nurses and nursing assistants as appropriate to clinic operations. Participate in nursing education, staff orientation, competency development, and quality‑improvement activities. Collaborate with the interdisciplinary team to improve holistic primary care services and promote optimal patient outcomes. Evaluate nursing practice using current evidence, provide constructive peer feedback, and contribute to an evidence‑based practice culture. Support a safe, professional learning environment and share clinical knowledge and educational insights with colleagues. Pay: Competitive salary, regular salary increases, and potential for performance awards. Paid Time Off: 50 days per year (26 days annual leave, 13 days sick leave, 11 paid Federal holidays). Retirement: Traditional federal pension (5 years vesting) and federal 401(k) with up to 5% VA contribution.
