Job Description
Job Title: LVN / LPN

Experience: Minimum of one year recent full-time paid clinical experience in an acute care health facility or ambulatory care facility preferred.
License: Current license as a Practical Nurse in the state of Illinois.
Certifications: BLS certification
Must-Have: Graduate from an accredited school of Practical Nursing in the state of Illinois.
Must have completed coursework for medication administration (Pharmacology) in accordance with state regulations.
Description:
The candidate must provide documentation of their qualifications if requested.
A minimum of one year of recent clinical experience in an acute care health facility is preferred.
Candidates are required to float within the facility as needed.
Every other weekend coverage is required, totaling two weekends per month.
There is a maximum of five days for requested time off, with no more than two consecutive days.
Requests for time off are not permitted on holidays or the days adjacent to holidays.
Vaccine requirements include proof of COVID-19 vaccination prior to the start date, with no exemptions allowed.
Proof of flu vaccination is required, with medical or religious exemptions only.
The facility requires compliance with all credentialing standards and procedures.
Candidates must be responsive and provide updates for all required documentation timely.
Floating outside of primary competencies may occur at the facility's discretion.
All clinicians may receive notification to float shortly before their shift.
Returning candidates must complete recredentialing and orientation if their break in service is less than 30 days.
Lab work must be completed promptly after accepting an assignment.
Candidates must adhere to all expectations outlined for compliance to continue their assignment.
