Registered Nurse (RN) | Home Dialysis

Location Avera Dialysis – Sioux Falls Work Type & Shift Regular. Primarily days with night, weekend and holiday on‑call involvement. Pay Range $32.50 – $48.50 per hour, actual rate based on experience. Position Highlights Possibly eligible for a $10,000 sign‑on bonus. Monday – Friday 8am – 5pm, with occasional nights, weekends, and holidays. On‑call involvement. May qualify for Avera’s Student Loan Repayment Program: an extra payment of $167 monthly, up to $10,000. Job Summary Provides major portions of self‑care and home dialysis training and monitoring for dialysis patients and/or their caregivers. Responsibilities Assess patient pre‑therapy to determine eligibility. Identify patient/family learning needs. Develop, implement, and evaluate an educational plan for patients. Provide patient education related to dialysis processes and procedures, including dialysis access care and management. Monitor and evaluate patients during a minimum of a monthly visit to problem‑solve, mentor, and ensure safe home therapy delivery. Problem‑solve and facilitate access to resources for patients on home therapy. Coordinate patient care with Medical Director/Nephrologist, patients, families, nursing staff, and ancillary departments. Maintain accurate documentation and use computer skills. Comply with departmental policies, scope of care, performance improvement activities, safety, environmental, and infection control standards. Document in the patient record according to policy and procedure. Promote compliance with state, federal, and regulatory agencies (CMS/ESRD, Quality Assurance, and Process Improvement efforts). Facilitate patient supply re‑ordering. Qualifications Registered Nurse (RN) with an active license in the state of practice. 1–3 years of nursing experience (preferred). Certified Hemodialysis Nurse (CHN), Certified Peritoneal Dialysis Nurse (CPDN), or Certified Dialysis Nurse (CDN) preferred. Specialty Certification in the American Society of Histocompatibility & Immunogenetics (ASHI) preferred. Dialysis or critical care experience preferred. Requires ability to perform essential job functions, including visual acuity, effective communication, hearing, and understanding speech.
