Physician Assistant (PA) – Advanced Wound Care & Senior Living Specialist Richfield, United States | Posted on 05/28/2026 Job Title: Physician Assistant (PA-C) | Senior Community Rounding Provider Compensation: $120,000 to $160,000+ Total Compensation (Base Salary + Productivity Incentive Program) + Mileage Reimbursement Benefits: Full Benefits Package, PTO, Paid Holidays, CME Allowance, Malpractice Coverage, DEA Reimbursement, and More Schedule: Full-Time | Monday through Friday Why Providers Are Drawn To This Opportunity $120,000 to $160,000+ total compensation opportunity through salary and productivity incentives. Serve as a key clinical decision-maker across multiple senior communities. Help coordinate care among physicians, nursing teams, therapists, families, and community partners. Build expertise in geriatrics, chronic disease management, preventative medicine, and post-acute care. Join a provider-led organization serving hundreds of senior communities throughout Minnesota. Receive mileage reimbursement, CME support, malpractice coverage, DEA reimbursement, PTO, and comprehensive benefits. What You'll Be Doing Provide on‑site primary care and rounding services throughout assisted living, memory care, and long‑term care communities. Perform assessments, chronic disease management, preventative care, and follow‑up visits. Develop and adjust individualized care plans. Coordinate care with physicians, nurses, therapists, caregivers, and family members. Support transitions of care and help reduce avoidable emergency room visits and hospitalizations. Ensure continuity of care across a diverse patient population. Qualifications Active Minnesota Physician Assistant (PA-C) license in good standing. Experience in geriatrics, family medicine, internal medicine, home health, hospital medicine, post‑acute care, or related settings preferred. Strong communication skills and confidence working collaboratively with interdisciplinary teams. Valid driver's license and reliable transportation. #J-18808-Ljbffr
