Job Summary:

Seeking a licensed Physical Therapist to provide outpatient home health services, including patient assessments and therapy plans in patients' homes or care facilities. The role involves traveling within a designated territory to deliver quality care and support patient recovery.

Responsibilities:

• 
  Evaluate patient conditions through comprehensive assessments and develop personalized treatment plans.
  
  
• 
  Deliver skilled therapy interventions to improve patient mobility and functional abilities.
  
  
• 
  Document patient progress and maintain accurate records within required timeframes using EMR systems.
  
  
• 
  Collaborate with patients, families, and healthcare teams to optimize care outcomes.
  
  
• 
  Travel up to 75 miles to patient homes and facilities, ensuring timely appointments.
  
  
• 
  Adhere to all compliance standards, including documentation and HIPAA regulations.
  
  

Qualifications:

• 
  Hold a valid state licensure as a Physical Therapist.
  
  
• 
  Have a minimum of 1 year of recent adult home health experience.
  
  
• 
  Possess current CPR and BLS certification.
  
  
• 
  Demonstrate proficiency in EMR systems such as HomeCare HomeBase, Oasis, or Outlook.
  
  
• 
  Be willing and able to travel throughout the assigned territory.
  
  
• 
  Must have reliable transportation, a valid drivers license, and auto insurance.
