Care Initiatives Hospice is seeking a compassionate Registered Nurse (RN) to join as an RN Case Manager. You will meet patients where they are—home, long-term care, assisted living, or hospital—and deliver comprehensive, patient-centered hospice care with dignity and respect. Responsibilities include conducting initial and ongoing assessments, developing and updating care plans under physician direction, coordinating with an interdisciplinary team, and providing education to patients and #J-18808-Ljbffr Care Initiatives
