About the Role We are seeking an experienced and compassionate Hospice PRN Case Manager who is dedicated to providing exceptional end‑of‑life care with dignity, empathy, and clinical excellence, and who is willing to travel in the DFW Area. This role is ideal for a nurse who is passionate about providing high‑quality, patient‑centered end‑of‑life care while enjoying flexibility in scheduling. You will work closely with patients, families, and an interdisciplinary team to ensure comfort, dignity, and support throughout the hospice journey. Service Areas Dallas Duncanville Mansfield McKinney Allen Key Responsibilities Conduct comprehensive patient assessments and develop individualized care plans Provide skilled nursing care and symptom management Coordinate care with physicians, social workers, chaplains, and other team members Educate patients and families on disease progression, medications, and care techniques Monitor patient condition and adjust care plans as needed Maintain accurate and timely clinical documentation Participate in on‑call rotation as needed (PRN basis) Ensure compliance with hospice regulations and standards Qualifications Current and unrestricted RN license in the state of Texas Minimum 1–2 years of hospice, home health, or related experience preferred Strong clinical assessment and critical thinking skills Excellent communication and interpersonal abilities Ability to work independently and manage time effectively Reliable transportation and valid driver’s license Current CPR certification (required) Benefits 401(k) Flexible schedule Opportunity for advancement Training & development Flexible PRN scheduling Competitive pay rates Supportive interdisciplinary team environment Opportunity to make a meaningful impact in patients’ lives Compensation The salary is Negotiable.
