Primary Care Medical Director

We are seeking anexperienced Primary Care Physician to serve as a Medical Director for a growingoutpatient physician practice in Lancaster, New Hampshire. This leadershipopportunity combines clinical practice with medical direction and is ideal fora physician interested in long-term leadership within a close-knit ruralcommunity. Position Highlights Full-Time Leadership + Clinical Role Medical Director: 16 hours/week Primary Care: 24 hours/week Approximately 20–24 patients per day Outpatient primary care EMR: Meditech Expanse + Dragon Community call, including local nursinghome patients, TBD Supported by an RN Team Leader anddedicated Medical Assistant Strong IT and marketing support Medical DirectorResponsibilities Provide clinical leadership andoversight for physician practice providers. Promote quality, safety, and consistencyof clinical care. Mentor and support providers andcoordinate medical staff education. Participate in practice planning,operations, and strategic initiatives. Collaborate with senior leadership,practice managers, and clinical teams. Develop and implement outpatientclinical policies and workflows. Support provider recruitment andonboarding. Address provider, patient, and staffconcerns and facilitate resolution of clinical issues. Monitor provider quality, utilization,productivity, and performance metrics. Lead quality improvement and value-basedcare initiatives. Serve as the practice EMR superuser andadvocate for technology solutions. Support controlled-substance bestpractices and initiatives addressing social determinants of health. Serve as chair of the physicianpractices and participate in appropriate medical staff and qualitycommittees. Make periodic site visits to practicelocations as needed. Primary CareResponsibilities Provide comprehensive outpatient primarycare. Evaluate and manage acute and chronicmedical conditions. Perform preventive care, wellnessvisits, and health screenings. Develop individualized treatment plansand coordinate specialty referrals. Maintain accurate and timelydocumentation. Provide approximately 21.5 hours ofdirect patient care weekly, plus documentation time.
