RN Level 2 Geriatric Wound Care

Job Summary:

The Registered Nurse (RN) Level 2 will provide skilled nursing care to geriatric long-term care residents, focusing on wound care without acute care or tracheostomy needs. The RN is responsible for direct patient care, assessments, documentation, nursing care plan implementation, medication administration, and oversight of LPN and CNA staff. Strong communication, flexibility, and delegation skills are essential. A minimum of two years' experience in a long-term care setting is required, along with an active New York State RN license and American Heart Association BLS certification. Responsibilities:

• Provide direct nursing care to up to 20 long-term care residents.
• Conduct patient assessments and document findings accurately.
• Implement and oversee individualized nursing care plans.
• Administer medications safely and according to protocols.
• Supervise and collaborate with LPN and CNA colleagues.
• Communicate effectively with patients, families, and interdisciplinary team members.
• Maintain compliance with facility policies and regulatory standards.

Required Skills & Certifications:

• Active Registered Nurse license in New York State only.
• Minimum of two years' experience in a long-term care setting.
• Current BLS certification from the American Heart Association.
• Strong clinical assessment and medication administration skills.
• Excellent communication, delegation, and teamwork abilities.

Preferred Skills & Certifications:

• Experience with telemetry monitoring.
• Previous supervisory experience in a long-term care environment.
• Flexibility to adapt to varied patient care needs and staffing requirements.

Special Considerations:

• Work includes days, evenings, and nights shifts.
• Weekend, holiday, and on-call coverage as needed per unit requirements.
• Care setting is a long-term care facility without acute care or tracheostomy patients.

Scheduling:

• Shift assignments include day, evening, and night shifts.
• Schedules are determined based on unit needs, including weekends and holidays.
• On-call duties are required according to unit staffing policies.
