Care Coordination SupervisorHybrid • Whitehall, OH 43213OverviewSalary Range $57,470.00 - $71,843.00 Salary Position Type Full Time Travel Percentage 20 - 30%DescriptionThe Buckeye Ranch is seeking a Care Coordination Supervisor to join our Care Management Entity (CME). The Care Coordination Supervisor directs the programmatic supervision of care coordinator teams and ensures the implementation of the specific program model. The supervisor is primarily responsible for ensuring adherence to high fidelity wraparound model of care coordination, ensuring children, youth and family/caregiver centered, high quality services, positive outcomes, and satisfactory business results for the program serving children and families/caregivers of a variety of socioeconomic, racial, ethnic, and cultural backgrounds.Learn about The Buckeye Ranch:For decades, The Buckeye Ranch has been restoring hope and providing healing for children, youth, and families in Central Ohio. With help from The Buckeye Ranch, they're empowered to overcome obstacles, tap into their resilience, and move forward toward healthy, fulfilling lives.What you'll do:Ensure the children, youth, and families/caregivers have a voice and choice in all coordinated care and services provided by care coordinator team members pertinent to completed ICC or the MCC HFWA certification training with the COE.Develop and implement training plans for care coordinators in their program and advocate on behalf of children, youth, and families/caregivers and within systems of care; with schools, community agencies, and other providers, focused on adherence to high fidelity wraparound principles and care.Provide supervision for direct report care coordination team members.Conduct group supervision/team meetings.Collaborate with the Utilization Manager on case assignment and other areas of production/impact.Monitor program budgets relating to the operation of the assigned programs.Collaborate with all relevant systems and key participants within systems pertaining to children or youth and family/caregiver being served to ensure the highest quality of services are being provided with focus on ensuring family voice and choice.Our benefits:Benefit effective date: First of the month following 30 days of employment.Wonderful medical, dental, and vision insurance.Paid Time Off (Three weeks within the first year)Company Paid Life InsuranceCompany Paid Short Term Disability401K + Employer ContributionNon-Profit Student Loan Forgiveness ProgramEmployee Assistance ProgramTuition AssistanceEmployee Referral ProgramQualificationsWho you are:You have a bachelor's degree in human services, or related field; master's degree preferred.You have two years of relevant working experience.You preferably have an active LSW or LPC.You have an active driver's license and proof of auto insurance.Applicants are considered for all positions in accordance with statutes and regulations concerning non-discrimination on the basis of race, ancestry, age, color, religion, sex, national origin, sexual orientation, gender identity, non-disqualifying disability, veteran status, or other protected classification.
