This Registered Nurse (Community Nursing Home Coordinator) position is in the Nursing Services at the Worcester, MA campus. This position is full-time at 40 hours per week. Duties include but not limited to: Responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care. Assesses, plans, implements, and evaluates care based on age specific components. Evaluates care and outcomes to ensure timely and appropriate provision of services. Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care. Follows procedures per established policies and guidelines. Influences care outcomes by collaborating with members of the interdisciplinary team. Executes position responsibilities that demonstrate leadership, experience, and creative approaches to management of complex client care Work Schedule: Monday - Friday 08:30 a.m. - 04:00 p.m. Telework: Yes, may be authorized on an ad-hoc basis at the supervisor's discretion.
