Licensed Practical Nurse

Licensed Practical Nurse – Sevita Location: Cleveland, TN | Employment Type: Full-Time (also part-time and PRN available) Pay: $20.40 hourly (Full-time) / $25.00 hourly (PRN) Schedule: Rotating 7:00 AM – 7:00 PM. Week 1: 3 days, Week 2: 4 days. PRN: 2 days/week and 1 weekend/month. Company D&S Community Services, part of the Sevita family, provides community-based services for individuals with intellectual and developmental disabilities. Responsibilities Monitor health conditions of individuals served and ensure quality services. Administer medications, provide documentation, and oversee staff medication administration. Provide prescribed medical treatments and personal care services. Conduct assessments and implement health‑care components in Individual Service Plans (ISP); respond to condition changes. Educate staff on universal precautions, infection control, and medication administration. Liaise with medical professionals and external agencies to coordinate consistent treatment. Provide training to individuals served on self‑medication, prescribed treatments, sex‑education/STD prevention, and smoking cessation. Qualifications Associate’s Degree in Nursing or related field, or state certificate. One year of nursing experience with the population served. Current state LPN or LVN license for state of residence. Valid driver’s license, car registration, and auto insurance. Required certifications per state regulations (e.g., CPR/FA, HIV/AIDS certification, BBP, ACT training). Effective communication with staff, individuals served, and families.
