Coordinator Patient Care Services (Registered Nurse) Acute Care For The Elderly/Headache Unit
With a commitment to patient- and family-centered care, the professional nurse uses the nursing process in ways that values the uniqueness of each individual and addresses the physical, psychological, emotional, and social needs of the diversity of patients served by Jefferson Health. Assists in leading the daily operations of a specific unit. Acts as a resource, lead, and clinical role model for the patient care team. Advances and oversees patient care standards of the unit for the designated shift. Also facilitates and coordinates patient care delivery and flow through the continuum of care. Supports nursing professional development, Magnet, and Shared Governance through participation in, and mentoring of colleagues, on councils and the Clinical Ladder program.
As an RN, Coordinator of Patient Care Services, you are responsible for the following:
- Acts as a resource, role model, and clinical leader for the patient care team.
- Evaluates patient needs and makes care recommendations based on those needs.
- Monitors staff schedules to ensure adequate staffing to meet patient care needs.
Job duties include:
- Interacting with co-workers, visitors, and other staff consistent with the values of Jefferson.
- Communicating with and supporting the interprofessional teams and services, patient, family and designated care partners.
- Collaborating with the medical staff to facilitate consistency of interdisciplinary plans of care to ensure quality patient care.
- Providing timely Smart Square schedules and on call schedules if applicable.
- Monitoring staff schedules to ensure adequate staffing to meet patient care needs and maintain quality patient care.
- Collaborating with the Nurse Manager to evaluate, maintain and improve patient care team member performance.
- Supporting nursing professional development, Magnet, and Shared Governance through participation in, and mentoring of colleagues, on councils and the Clinical Ladder program.
Minimum qualifications include:
- Specialized diploma graduate of an accredited school of nursing required.
- 3 years as a registered nurse in an acute care setting.
- Effective communication skills.
- Patient-and family-centered practice knowledge.
- Cultural awareness.
- Care coordination knowledge.
- Patient safety knowledge.
- Medication administration and prescription knowledge.
- Discharge planning knowledge.
- Assessment and diagnosis knowledge.
- Health information documentation knowledge.
- BLS - Basic Life Support - American Heart Association.
- RN - Licensed Registered Nurse_PA - State of Pennsylvania.
- RN - Licensed Registered Nurse_NJ - State of New Jersey.
Preferred qualifications include:
- Bachelor's degree BSN or completion of BSN or appropriate advanced degree within 3 years of hire strongly preferred.
- Certification in specialty.
- If certification is required on a specific unit; certification must be obtained within six months of hire date.
Physical demands include lifting and carrying 40-50 lbs. Examples: Push/pull patients on bed, stretcher (requires 29 lbs. push force), lateral transfers up to 50 lbs. of the patient's weight. Frequent to continuous standing/walking. Patient transporters can walk 8-10 miles per shift. *Patient care providers may be required to perform activities specific to their role including kneeling, bending, squatting and performing CPR.
