Job Description
Registered Nurse (RN) - Home Health Job Description We are seeking a dedicated Registered Nurse (RN) to coordinate and provide home nursing care based on physician orders and agency policies. The RN will be responsible for applying the nursing process competently and supervising Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses. Key Responsibilities
- Coordinate and direct home nursing care according to physician orders.
- Implement the nursing process through assessment, planning, and evaluation.
- Supervise and evaluate the performance of Home Health Aides and LPNs/VN.
- Conduct home visits, providing care to an average of 5-10 patients per day.
- Engage in periodic office meetings and collaborate with the clinical team.
- Complete all patient documentation within 24 hours of visits.
- Adhere to agency policies and maintain compliance with EMR systems.
- Minimum of 1 year of experience in Adult Home Health or Hospice care.
- Current RN license in hand; no pending licenses will be accepted.
- Proficient in OASIS and HomeCare HomeBase (HCHB) EMR systems.
- Current certifications in CPR and BLS are required.
- Must have a valid driver's license, a reliable vehicle, and auto insurance.
