Clinical Case Manager/Educational Liaison

Clinical Case Manager/Educational Liaison (CCM) – Residential Services NorthEast Center for Youth and Families, Inc. is hiring a full‑time CCM to focus on clinical, collateral case management, ongoing assessments, and aftercare. The CCM will also serve as the Educational Liaison to support youths’ learning and ensure their IEPs are comprehensive and meet their needs in the school environment. Responsibilities Act as an advocate for youth in the educational setting. Be available during school hours, attend school meetings, and maintain relationships with students during after‑school hours. Provide needs assessment that emerges and offer after‑hours on‑call support to programs. Coordinate ongoing assessments, case management, and aftercare services. Qualifications Master’s Degree. Successful completion of required trainings once hired, including but not limited to CPI, CPR, First Aid. Approved CORI & DCF background checks (performed by the EEC). Approved driving background check. Valid driver’s license and reliable transportation. Benefits We offer paid training for all employees.
