HawthornChildren’sPsychiatricHospitalusestheSanctuaryModeltoguideourinteractions with our teammates and the care we provide to our consumers to promote healing, safety, and nonviolence.This is accomplished byfollowing the foundationsof the Sanctuary Model to provide trauma informed and responsive care. ConductcomprehensiveBiopsychosocialAssessments(Initialand Annual) Completeadmissionanddischarge paperwork. Provideindividual and family therapy and lead therapeutic groups, completing corresponding documentation. Writemonthly/weeklysummaryprogressnotesfor clients. Providecasemanagement servicestoassigned clients and their families/guardians including discharge planning. Conductsuicideriskandotherriskassessmentsas needed. Collaboratewithaninterdisciplinaryteamonaroutinebasistoprovide, document,andorient treatment. Participateindischargeandlegalprocessesas necessary. Disseminatepertinentinformationtovesteddepartmentswithinandoutsidethe facility. Otherdutiesas assigned. Priorexperienceprovidingservicesfor individuals, preferably youth and families, with severeandpersistentmental health conditions WorkingknowledgeofDSM,evidence-basedpracticesandsocialworkethicsarestrongly encouraged. Degreeand/orLicenseinSocialWork(MSW,LMSW,orLCSW). LCSWpreferred butwill consider candidates who meet the minimum qualifications.
