Job Description

Are you passionate about helping individuals and families build a path toward stable housing? As a Housing Case Manager (Case Navigator) in Galveston, you'll play a vital role in connecting clients experiencing housing instability with the resources and support they need to achieve long-term success. This rewarding position combines case management, advocacy, and community collaboration in a mission-driven environment where your work makes a meaningful impact every day.

Responsibilities

• Conduct client assessments to determine eligibility, identify needs, and develop individualized housing stability plans.
• Connect clients with housing resources, employment services, financial assistance, and other community programs.
• Maintain accurate case documentation and client records within HMIS and other required systems.
• Conduct follow-up appointments, including office visits and home visits, to monitor client progress and provide ongoing support.
• Collaborate with community partners and internal teams to ensure clients receive comprehensive services.

Qualifications and Requirements

• Bachelor's degree in Social Work, Psychology, Human Services, or a related field required.
• Minimum of two years of case management experience in housing, homelessness, or social services.
• Experience working with diverse populations using a trauma-informed and client-centered approach.
• Strong communication, organization, documentation, and Microsoft Office skills.
• Valid Texas driver's license and reliable transportation required.

Your final compensation will depend on your skills, qualifications, experience, location, and internal pay equity.
