You will collaborate with physicians, families, and interdisciplinary team members to provide high-quality, individualized care.Key ResponsibilitiesPerform patient evaluations and develop individualized treatment plansProvide therapy for communication, cognition, and swallowing disordersMonitor patient progress and adjust treatment plans as neededMaintain accurate and timely clinical documentationCommunicate effectively with physicians, residents, families, and the care teamParticipate in care planning and discharge planningProvide education and training to staff and caregivers as neededQualificationsDegree in Speech-Language Pathology (Master’s or higher preferred)Active, unencumbered SLP license in South Carolina (required)Experience in a skilled nursing or rehab setting preferredFamiliarity with PointClickCare (PCC) and Casamba preferredStrong clinical, communication, and organizational skillsCommitment to patient-centered careWhy Join Cherrydale Post Acute?At Cherrydale, you’ll enjoy the flexibility of PRN work while being part of a supportive team that values your expertise and contributions.
