Responsibilities

• Perform initial physical therapy evaluations and ongoing progress assessments for residents in a long-term care setting.
• Develop and implement individualized treatment plans aimed at improving mobility, strength, balance, gait, and functional independence.
• Deliver evidence-based interventions such as therapeutic exercises, gait training, manual therapy, and neuromuscular re-education as appropriate.
• Monitor and document resident progress, adjust goals as needed, and communicate changes to the interprofessional care team.
• Collaborate with nursing, occupational therapy, speech-language pathology, and social work to coordinate care and discharge planning.
• Educate residents and families on home exercise programs, safety, fall prevention, and device or equipment use as part of ongoing recovery.

Requirements

• Experience in a skilled nursing or geriatric long-term care setting preferred
• Ability to work a full-time 8:00 a.m. – 4:30 p.m. schedule during a 13-week assignment
• Strong documentation, organizational, and communication skills
• Comfortable functioning within a multidisciplinary care team and a regulatory environment
• Willingness to provide patient- and family-centered care for an aging population
• Availability to begin on 07/13/2026 and commit to the contract duration
