Job Type: Full-Time, On The Road As an Admissions RN at Moments Hospice, you will join a team dedicated to making our patients' final days, weeks, and months as comfortable as possible. The role offers autonomy and variety in hospice care, with a supportive community and fleet car benefit covering gas and insurance. Hours Monday – Friday, 12:00 pm – 8:00 pm Responsibilities Participate in information gathering, analysis, education, and form completion aspects of the admissions process Evaluate patients for hospice appropriateness, including medical records review as requested Obtain agreement for admission of referred patients and families who meet admission criteria Analyze data collected to facilitate or obtain admission orders Build effective referral source relationships to support high‑quality and responsive care expectations Maintain clinical and professional competence Initiate and develop effective relationships with appropriate admissions and patient‑care resource personnel Utilize interpersonal skills to educate and address any questions the patient or patient’s family has concerning hospice and palliative care Requirements Valid Registered Nurse (RN) license Driver's license, appropriate insurance, and access to a vehicle 1–2 years of nursing experience (preferred) Utmost compassion and integrity Current RN licensure in the state where duties are performed Experience in hospital, physician practice, long‑term care, or assisted living facility settings Strong presentation skills Previous sales or service experience preferred Benefits Competitive salary Company car with fuel and insurance covered Generous PTO accruing immediately Comprehensive health, dental, and vision insurance 401(k) with company matching We may use artificial intelligence (AI) tools to support parts of the hiring process, including reviewing applications, analyzing resumes, or assessing responses.
