Nurse Practitioner - Hybrid Role

Job Summary:

We're seeking an experienced Nurse Practitioner (NP) or Physician Assistant (PA) with a background in Geriatrics or Internal Medicine to deliver high-quality preventative care to residents in skilled nursing facilities. As a hybrid role, you'll have the flexibility to work on-site and remotely, supported by a dedicated scribe team for efficient documentation.

Responsibilities:

• Build strong relationships with facility staff and patients while maintaining a patient-centered approach

• Collaborate with a multidisciplinary team to provide comprehensive care

• Conduct on-site visits and remote work from home

• Document patient care efficiently with the support of a dedicated scribe team

• Participate in education and training programs to stay up-to-date with industry developments

Requirements:

• Active NP or PA license in the state of practice

• 1+ year of experience in Geriatric care and documentation review

• Excellent communication, collaboration, and teamwork skills
