Physical Therapist - Home Health Full Time, Days

Location: White River Junction, VT area; Lebanon, NH area; Bradford, VT area; Windsor, VT area

Assignment Duration: This is a 13-week contract position.

Schedule: Full-time, Day shift

Job Description: As a Physical Therapist with Visiting Nurse and Hospice for Vermont and New Hampshire, you will have the opportunity to form strong relationships with your patients, becoming an integral part of their care team. In this role, you will have the autonomy to assess and provide therapeutic interventions while working closely with an interdisciplinary team.

Responsibilities:

• Evaluate the patient's need for physical therapy and develop an appropriate treatment plan.
• Implement treatment programs in accordance with agency policies and professional standards of care.
• Communicate effectively with all members of the health care team.
• Document evaluations, treatment goals, and objectives accurately.
• Educate patients and family members on the rationale for treatments and procedures.

Required Experience / Certifications / Licensure:

• Bachelor's degree in Physical Therapy.
• Must be a licensed Physical Therapist in Vermont and New Hampshire, or hold an unencumbered compact license.
• One year of experience as a Physical Therapist in a home healthcare setting.
