Registered Nurse (RN) Care Coordinator - Total Joint Program


Full-Time | 40 Hours per Week | 5 Days per Week

Position Summary


We are seeking a compassionate and highly organized Registered Nurse (RN) Care Coordinator to serve as the primary coordinator for patients undergoing total joint replacement procedures. This important role provides education, care coordination, and ongoing support throughout the patient's surgical journey-from pre-operative planning through post-operative recovery.


The RN Care Coordinator partners closely with patients, families, surgeons, providers, and interdisciplinary team members to ensure seamless care, promote positive outcomes, and deliver an exceptional patient experience. This position is ideal for an RN who enjoys building meaningful relationships, advocating for patients, and collaborating across departments to enhance care delivery and program success.

Key Responsibilities

• Coordinate care for patients undergoing total joint replacement surgery.
• Provide individualized patient and family education before, during, and after surgery.
• Guide patients through pre-operative preparation and recovery planning.
• Serve as a primary point of contact for patients throughout the surgical process.
• Collaborate with surgeons, providers, nursing staff, rehabilitation services, and other interdisciplinary team members.
• Monitor patient progress and facilitate appropriate follow-up care.
• Promote patient engagement, satisfaction, and successful clinical outcomes.
• Advocate for patient needs while ensuring a smooth and coordinated care experience.
• Support quality improvement efforts and program initiatives within the Total Joint Program.

Qualifications

Required

• Graduate of an accredited nursing program.
• Current Registered Nurse (RN) license in the State of Nebraska.
• Strong communication, organizational, and patient education skills.
• Ability to work collaboratively with patients, families, providers, and multidisciplinary teams.

Preferred

• Experience in orthopedics and/or total joint replacement care.
• Previous care coordination, case management, or patient navigation experience.
