Job Title Hospice Admissions Registered Nurse Location Allen Park, MI, USA | Troy, MI, USA (additional location) Job Summary Residential Home Health and Hospice seeks a dedicated Hospice Admissions RN to collaborate in managing and delivering patient care plans for patients in Downriver cities and surrounding areas. The role follows a 36‑hour weekly schedule: Monday, Thursday and Friday from 10am to 10pm. Successful completion of the first year can earn a $10,000 success bonus. Responsibilities Collaborate with the hospice admissions team to manage and deliver patient care plans. Provide admission assessments, coordinate care transitions, and maintain accurate documentation using EMR software. Travel to patient locations up to 20% of working time, ensuring timely charting and compliance with critical deadlines. Utilize mobile devices (tablet, smartphone) for real‑time communication and documentation. Qualifications Valid RN license in the state of employment. Associate degree or higher from an accredited School of Nursing. Minimum two years of RN experience; hospice experience preferred. Current driver’s license and ability to travel as required.
