Key Responsibilities\n

  • Conduct thorough psychosocial assessments to determine patients' mental health needs, strengths, and challenges, helping to formulate individualized care plans.

  • Deliver evidence-based therapeutic interventions, including cognitive-behavioral therapy and dialectical behavior therapy to address the unique mental health needs of patients in hospice.

  • Work closely with an interdisciplinary team, including psychiatrists and nurses, to ensure comprehensive and coordinated patient care.

  • Guide patients and their families through understanding diagnoses, treatment options, and available resources, empowering them during critical decision-making processes.

  • Facilitate individual, group, and family therapy sessions focused on coping strategies, grief processing, and emotional support.
  • \n
  • Develop treatment plans that not only meet clinical standards but also align with patients' personal goals and preferences, ensuring that their voices are central in their care.

  • Assist patients in navigating the often-complex mental health system, advocating for their rights, needs, and access to necessary services.

  • Lead therapeutic groups, psychoeducational workshops, and discharge planning meetings to solidify patient support networks.

Patient Population / Client Focus\n

This role is dedicated to serving patients receiving hospice care and their families. You will work with individuals facing terminal illnesses, addressing not only the emotional and psychological impacts of their diagnoses but also assisting families in coping with grief and loss. This specialized focus requires sensitivity and proficiency in handling complex emotional dynamics associated with end-of-life care and bereavement.

Qualifications\n

  • Master's degree in Social Work from an accredited institution.

  • Active state licensure as a Licensed Clinical Social Worker (LCSW).

Compensation and Benefits\n

We are pleased to offer a competitive salary range of $80,000 to $104,000 annually, commensurate with experience. Additional benefits include:
