Occupational Therapist - Full Time

Position Overview Provides occupational therapy services in a home health setting, focusing on assessment, treatment planning, and delivery of care to help patients achieve independence and safety. Key Responsibilities Conduct initial and ongoing assessments of mobility, coordination, strength, and functional abilities. Administer and interpret tests to establish goals and outcomes. Develop written evaluations and plans of care outlining needs, goals, and progress. Deliver occupational therapy services per physician orders, maintaining documentation and modifying treatment plans as needed. Manage discharge planning, including patient education on home exercises. Coordinate with healthcare team members, including physicians, nurses, and caregivers. Supervise other personnel (e.g., OT Assistants, Home Health Aides) as applicable. Complete assessments in compliance with regulations where permitted. Qualifications & Skills Degree in Occupational Therapy from an accredited program. Current, unrestricted state license as an Occupational Therapist (WI and MN). Valid CPR certification and driver’s license.
