Job Description Develops and coordinates a clinical treatment program for diagnosis and treatment of residents presenting with moderate to severe mental, emotional, or social problems Conducts and documents mental health evaluations Conducts initial and continued assessments, containing psychosocial histories, for accurate case conceptualization and treatment planning for the effective delivery of psychotherapy and counseling services Develops a detailed psychiatric crisis intervention plan when appropriate Conducts individual, family, and/or group therapy sessions; use empirically supported interventions appropriate to the phase of the treatment plan and the client’s stage of change Documents individualized treatment plans and subsequent treatment plan reviews with plan amendments; continued documentation of the clients' progress toward the solidified goals including provisions of support and intervention. Additional documentation shall include the client’s response to implemented interventions, client status during each visit, and specific details supporting the continuation or discontinuation of services following each session Delivers appropriate emergency interventional services with clients including crisis intervention, consultation, resource referral, and education Provides timely documentation, according to current policy and procedure, via the electronic medical record consistent with the services delivered (i.e. evaluation, assessment, or therapy session) Maintains professionalism consistent with legal standards and professional (NASW) Code of Ethics May be in charge with developing, coordinating, and evaluating a specific program or service and other duties as assigned Admission and discharge planning Benefits Competitive salary Medical, Dental & Vision plans Vacation Plan Paid Time Off 401K plan Tuition Reimbursement Internal growth opportunities Positive work environment Regular staff appreciation and recognition Qualifications Bachelor’s Degree in Social Worker 1 year of experience working with intellectual disabilities, preferred PASRR experience preferred Pillar of Cedar Valley Nursing and Rehabilitation is an equal opportunity/affirmative action employer.
