Physical Therapist

This position involves providing direct patient care to home care or home health patients based on physician orders, including assessment, treatment planning, and supervision of physical therapy practices. The role requires instructing patients and families on treatment procedures, assisting with physical activities and device use, and re-evaluating patient progress to optimize outcomes. Collaboration with physicians and other practitioners is vital to integrate care effectively. Additionally, the therapist evaluates, fits, and adjusts prosthetic and orthotic devices, recommending modifications as needed.
