• Assist clients with activities of daily living (ADLs), including bathing, grooming, dressing, toileting, and personal hygiene.

• Support safe mobility, transfers, positioning, and range-of-motion exercises as directed by the care plan.

• Help with light housekeeping, laundry, meal preparation, feeding, and grocery or errand assistance.

• Provide medication reminders in accordance with agency policy and the client care plan.

• Monitor and report changes in the client's condition, behavior, or home environment to the supervising nurse.

• Offer companionship and emotional support, engaging clients in conversation and approved activities.

• Accurately document care provided, observations, and hours worked on required forms or the agency's electronic system.

• Follow the individualized plan of care and adhere to all infection-control, safety, and HIPAA privacy standards.

• Active New Jersey CHHA certification in good standing with the New Jersey Board of Nursing.

• Valid CPR certification preferred (or willingness to obtain).

• Successful completion of a criminal background check and required pre-employment health screenings (including TB test).

• Ability to read, write, and communicate effectively in English.

• Reliable transportation and the ability to travel to assigned client homes.

• Physical ability to lift, transfer, and assist clients, and to stand, bend, and move throughout a shift.

• Compassionate, dependable, and professional, with strong interpersonal and observational skills.

• Prior home care or caregiving experience is a plus but not required.
