LTSS Care Manager & Waiver Services Coordinator

Molina Healthcare is seeking a Care Coordination Specialist to support LTSS programs and member care across the continuum. The role collaborates with a multidisciplinary team to ensure high-need members progress toward outcomes and cost-effective care. Responsibilities include assessing, planning, monitoring, and coordinating waiver services, with anticipated 25–40% local travel.
