Licensed Practical Nurse Home Health McDowell

CarePartners is seeking a compassionate and skilled Licensed Practical Nurse (LPN) for our Home Health team in McDowell. In this field‑based role, you will deliver high‑quality, patient‑centered nursing care to adults and seniors in their homes, supporting CarePartners of Connecticut’s mission to provide coordinated, accessible healthcare through its network of partnered providers. As an integral member of our Healthcare & Medical Services department, you will conduct comprehensive patient assessments, monitor vital signs, provide wound care, administer medications and treatments, and document all care accurately in the electronic health record. You will collaborate closely with physicians, registered nurses, therapists, social workers, and care coordinators to ensure each patient’s plan of care is followed, updated, and optimized for safety, comfort, and improved health outcomes. The LPN Home Health Nurse will educate patients and their families on disease management, medications, home safety, and self‑care techniques, promoting independence and helping to prevent unnecessary emergency visits and hospitalizations. You will be responsible for communicating changes in condition promptly, participating in care conferences, and supporting smooth transitions of care across settings.
