Job Title: Personal Care Assistant Job Summary Join our dedicated team at the Client day center in San Jose, where you will play a crucial role in delivering personal care services to our participants. As a Personal Care Assistant, you will support various departments, including Nutrition, Engagement, and Home Health, ensuring a safe and engaging environment for all. Collaborate with colleagues and stakeholders to uphold the Client values, team culture, and mission. Job Responsibilities Provide personal care and assistance with ADLs (activities of daily living) to participants, including escorting, toileting, bathing, dressing, grooming, and meal assistance. Measure and document participants' vital signs as instructed. Assist participants with boarding and exiting WelbeHealth transportation vehicles and act as a medical escort when needed. Ensure participant safety by observing activity on the day center floor. Assist with meal service, ensuring participants receive correct meals and provide feeding support when necessary. Assist participants with safe transfer in and out of their homes and act as a medical escort to appointments as assigned. Maintain personal care supplies such as incontinence supplies and personal grooming items. Support the Engagement team with participant activities. Assist the Home Care team as needed. Clean participant areas in the day center between meals and activities. Update the supervisor or any member of the IDT promptly of any changes in participants' condition. Comply with legal and company policies, maintaining privacy and confidentiality. Complete all required documentation, training, and occupational health screenings in a timely manner. Exercise flexibility in performing assignments as business needs evolve. Other duties as assigned. Required Skills & Experience Experience with electronic medical record systems. Minimum of one year of experience providing personal care to a frail or elderly population. Ability to follow directions from supervisors and communicate needs effectively. Excellent organizational and communication skills. Ability to work independently with minimal supervision. Demonstrated ability to prioritize in a fast-paced environment. Experience and competency working with people from diverse backgrounds and cultures. Commitment to unlocking the full potential of our most vulnerable seniors. Education & Certifications High School Diploma or Equivalency (if not certified). Required: CA Food Handler's Certificate. Preferred: Certification of Home Health Aide (HHA) or Certified Nursing Assistant in California.
