Department/Unit Summary Join our team in Inpatient Rehabilitation where you will provide compassionate care to patients alongside a dedicated team. You will play a crucial role in assessing patient needs, administering treatments, and ensuring patient comfort and safety. This position offers the opportunity to work in a collaborative environment focused on delivering high‑quality patient care How you’ll contribute As supervised and delegated by a registered nurse, provides direct patient care in accordance with applicable scope and standards of practice. Reports to the Director or Manager of Department. Essential Functions Contributes to the assessment of patients by collecting data for analysis by an RN. Performs plan of care interventions, including specimen collection and wound care; may administer medication. Documents patient care provided. Assists RN with development of plan of care. Assists patients with daily living activities including personal hygiene, elimination, nutrition, and ambulation. Monitors, documents, and communicates patient status and responses to interventions with RN, physician, patient, and family. Advocates for the physical and emotional well‑being of the patient. Participates in performance improvement and continuing education. Provides quality patient care and works flexible shifts/units as needed. Maintains regular and reliable attendance. Additional requirements include: Education: Graduate of accredited LPN program. License: / Certifications : Current LPN license in state of practice Basic Life Support certification is required within 30 days of hire.
