Home Health Nurse

Schedule: M-F

Assignment Details:

• Guaranteed Hours: 40
• Contract Weeks: 12

Requirements:

• Graduate of an accredited school of nursing
• Current licensure in state
• CPR certification
• One (1) to two (2) years of acute care experience in an institutional setting preferred
• Knowledge of Oasis
• Excellent observation, verbal and written communication skills
• Problem solving skills
• Basic math skills

Roles & Responsibilities:

• Conduct initial assessments of patients and families to determine home care needs and develop a personalized care plan.
• Regularly re-evaluate patient nursing needs and revise the care plan as necessary based on patient status.
• Administer medications and treatments as prescribed.
• Provide health care instructions and counseling to patients and families regarding their nursing needs.
• Act as Case Manager when assigned, coordinating care for an assigned caseload.
• Communicate changes in patient conditions to the primary physician and coordinate care with community health personnel.
