We are looking for a passionate clinician to join our team as a Care Coordinator, serving as a trusted clinical resource to hospitals, skilled nursing facilities, rehabilitation centers, physicians, and other healthcare partners throughout the community. This is not traditional sales, this is clinical leadership, patient advocacy, and relationship-building at its finest. Responsibilities This position will focus on the Mayo Clinic & Hospital Build meaningful relationships with hospitals, physicians, skilled nursing facilities, and community health care partners Assist patients and families during critical transitions from hospital or post-acute settings into home health care Serve as a trusted clinical resource for referral partners Educate healthcare professionals on the benefits and services available through home health Help remove barriers to care and improve continuity of care Advocate for patients and ensure seamless care transitions Collaborate with clinical and operational teams to support growth and exceptional patient experiences Why Clinicians Love This Position Use your healthcare knowledge and expertise every day without providing direct patient care. Qualifications We are seeking clinicians who: Have a passion for patient advocacy Enjoy connecting with people and building trust Communicate confidently with healthcare professionals Think strategically and solve problems proactively Are self‑motivated, driven, and goal‑oriented Want to expand their influence beyond direct patient care Must be a graduate of an approved school of nursing or therapy. Must be licensed in the state where they currently practice. Must have two (2) years of demonstrated field experience. Must have demonstrated experience and understand federal, state, and local laws and regulatory guidelines governing the operations of Medicare certified home health and hospice. Must have basic demonstrated technology skills, including operation of a mobile device. Education and experience, preferred Preferred Clinical Backgrounds Registered Nurse (RN) Licensed Practical Nurse (LPN) Physical Therapist (PT) Physical Therapist Assistant (PTA) Occupational Therapist (OT) Certified Occupational Therapy Assistant (COTA) Three (3) years of field experience is preferred. Previous experience in home health or healthcare sales is preferred.
