Job Title: Level 2 Registered Nurse Case Manager Profession: Case Manager RN Specialty: Case Manager RN Duration: 13 weeks Shift: Monday to Friday Hours per Shift: 8 hours Experience: - One year of experience as a clinical nurse in a hospital required - 9 months of experience in home care preferred License: - RN License (Client License ONLY) Certifications: - BLS Must-Have: - Certified Home Health Care Experience - OASIS Experience - Telemetry Experience Description: Develop and revise treatment plans and discharge plans in conjunction with patients, families, physicians, care delivery teams, and third-party payors. Complete and submit OASIS assessments, reassessments, transfers, resumptions of care, discharges, and significant changes in condition in accordance with defined time frames. Process orders and notify physicians of patient needs and changes in condition. Provide an environment that promotes respect for patients, their privacy, confidentiality, and property. Initiate appropriate preventive and rehabilitative strategies. Provide services requiring substantial specialized nursing skills. Consult with and educate patients, families, and other team members regarding disease processes, self-care techniques, and preventive strategies. Supervise LPNs to ensure patient health, safety, and compliance with the plan of care. Direct, coordinate, evaluate, and supervise the quality of patient care services provided. Interact with patients, physicians, referral sources, and others in a manner conducive to positive relationships. Support care management philosophy, including collaborative customer-focused planning and case management designed to meet individual health and service needs. Ensure the completion of all appropriate clinical records for compliance with legislation and seamless service delivery. Maintain current knowledge of available community-based services. Demonstrate flexibility and willingness to visit patients based on need. Ensure problem resolution at the point of contact. Ensure authorization and release of information forms have been signed by the patient or responsible party. Attend patient care conferences on assigned patients. Monitor results, progress, and take corrective action on quality indicators, including customer satisfaction. Understand the RN function's impact on patients, other care providers, readmission rates, and overall productivity. Pursue efforts to reduce or eliminate avoidable costs and errors. Respond to findings of various quality management reviews and ensure resolution of potential problem areas. Participate in peer review and quality assessment and performance improvement as assigned. Maintain productivity requirements. Ensure that appropriate visit authorizations are in place and that visit documentation reflects the medical necessity for care in the home setting. Promote recognition, respect, and celebration of workforce diversity. Participate in education, including orientation, in-service training programs, and self-development as needed. Demonstrate commitment to professional growth and competency. Be responsible for key performance indicators established by management. Perform other duties as requested.
