Guaranteed Hours: 40

Current state license as a Registered Nurse

Homecare Homebase Software Experience

OASIS Experience

CHPN certification (Preferred)

Proof of current CPR certification

Current driver's license

Reliable transportation and insurance

1-2 years experience as a Registered Nurse in a clinical care setting

2+ years Home health experience

Assess home health and/or hospice patients to identify their physical, psychosocial, and environmental needs.

Develop, document, and evaluate the Plan of Care to ensure quality and continuity of care.

Educate patients and/or their families regarding care, medications, home safety, and self-care practices.

Supervise and provide clinical directions to support staff to ensure quality service delivery.

Arrange referrals to appropriate agencies as needed.

Provide effective communication and serve as a liaison with patients/families, team members, and other healthcare professionals.

Must meet ongoing mandatory continuing education requirements.
