Overview
Office Location: Haven Hospice Lake City
Coverage area: 5 counties, approx 2800 square miles
Schedule: FT Monday - Friday, 8a-5p. No Weekends or Holidays

As a RN Case Manager You will:
- Assess/monitor physical, emotional, and psychological needs of patients
- Create hospice care plans that align with the patient's wishes and goals
- Direct nursing care: administering medications, treatments, and interventions
- Provide pain and symptom management
- Educate and support the patient’s family and caregivers
- Collaborate with interdisciplinary team
- Maintain accurate and timely documentation
