You will work closely with our interdisciplinary team to develop and implement individualized treatment plans that address the unique needs and goals of each resident. Physical Therapist (PT) Responsibilities: Conduct initial and ongoing assessments of residents to identify physical therapy needs and develop individualized treatment plans Design and implement effective physical therapy interventions, including exercises, stretching, and other therapies, to promote optimal function and independence Collaborate with the interdisciplinary team to develop and implement care plans that address the unique needs and goals of each resident Provide education and training to residents, families, and caregivers on physical therapy techniques and home exercise programs Participate in family conferences and provide regular updates on resident progress Maintain accurate and detailed records of resident treatment plans, progress, and outcomes Physical Therapist (PT) Requirements: Graduation from an accredited Physical Therapy program Current Physical Therapy license in the state of Massachusetts Ability to work in a fast-paced environment and prioritize multiple tasks and residents #J-18808-Ljbffr Brush Hill Care Center
